Prominent nipples can occur normally, but often cause embarrassment. A procedure under local anesthesia can produce a subtler nipple size and projection. The opposite problem, inverted nipples, affects a number of women but can also be improved with a small office procedure. For both types of procedures, trade-offs include interference with breast-feeding and possibly sensation. Implants have been used successfully since the 1960’s, so there are large numbers of women who encounter the need to redo the surgery. Implants may not last forever. Maintaining a result may eventually mean replacing implants the have worn out, correcting the effects of gravity and time, or correcting reactions such as capsular contracture (more common with gel implants).
We assist women in these situations with a variety of revision procedures, including: saline implant replacement, gel implant removal, removal of gel leakage, capsulectomy, submuscular repositioning, and/or mastopexy. For a complete assessment, Dr. Graham requests that you contact your previous surgeon or hospital for copies of your implant information and operative notes. We’ll discuss alternatives at your consultation.
The procedure will be done in one to three hours under general anesthesia. Medications will treat post-op pain. You’ll resume light activities within days and exercise in three weeks. Risk are listed and discussed prior to surgery. Results are long lasting, but breasts may change with age and gravity. Some patients consult our practice to correct problems with the position of their implants. These may have been placed well, but shifted over time, or the patients may have been dissatisfied with the initial or subsequent result from their original surgeon. Dr. Graham usually encourages patients to work with their surgeon, but realizes he or she may be unavailable, retired, or relocated. The variety of techniques includes revising the current capsule space or repositioning the space and replacing the implants. Most problems can be improved, but surgery cannot guarantee a particular result or exact match.
Expectations and risks will be discussed preoperatively. Patients receive written care and follow-up instructions. The recovery period is similar to other breast procedures, including the return to light activities within a few days after surgery and the return to more strenuous activities within a few weeks.
