The most common facial surgery procedure performed at our practice is treatment of the eyelids (sometimes referred to as blepharoplasty, eye lift, or eyelid tuck). The first facial lines and wrinkles typically appear around our eyes. Age and genetics are primary causes, but sun damage, swelling, and allergy-related conditions all contribute. Makeup, skin treatment programs, chemical peels, laser treatments, and BOTOX® Cosmetic all address some of the issues that arise, but eyelid surgery can also help improve the appearance of the areas around your eyes. During your consult we’ll review your medical history, including any eye problems you may have and medications you may be taking. Through eyelid surgery we try to correct loose skin on the upper and lower eyelids, as well as excess fat around the eye. Loose skin on the upper lid may interfere with smooth makeup application, fill the space above the eyelid, hang down covering much of the eyelid, or make one look tired or inattentive. Skin on the lower eyelid may drape loosely below the eye or accentuate texture and expression lines. Fat pads may cause the bags under the eye and may protrude above the upper lid as well.
The patient will be under local or general anesthesia during this out-patient procedure, which lasts between one and three hours depending on the extent of the procedure. Typical recovery supplies are provided to you. The pain is usually mild and easily controlled. Specialized post-operative massage techniques and makeup advice are usually provided by our Skin Care Specialist. Light activities can be resumed in four to seven days. The amount of discoloration in the treatment area varies, but usually resolves in one to three weeks.
The risks associated with the procedure are detailed in writing and discussed with you prior to surgery. Results last for years. Patients occasionally choose to maintain results by repeating the procedure again within seven to fifteen years. Brow lift (forehead lift) surgery can restore a youthful appearance by smoothing wrinkles across the forehead, elevating droopy eyebrows, and reducing excess skin above the upper lids. We offer both endoscopic brow lift and traditional brow lift surgery. Satisfied brow lift patients at our practice report looking more refreshed and alert.
The endoscopic brow lift is performed with a tiny scope and miniature camera. Two incisions less than one-inch long are made behind the hairline above the forehead, and two somewhat longer incisions are made in the temples. The layers that hold the brow in its low position are separated and repositioned and the muscles that cause frown lines to appear are selectively weakened. The lift is supported by hidden internal sutures, and the incisions are covered by your hair.
Another option is the traditional open brow lift, which Dr. Graham uses infrequently. It can improve deeper forehead lines and low hairlines, but entails a longer incision and more numbness than endoscopic brow lift. Dr. Graham will discuss the risks of each type of brow lift procedure with you before surgery.
The so-called chemical browlift is done with selective use of BOTOX® Cosmetic to weaken muscles that pull down portions of your eyebrows.
During your consultation, you’ll see before-and-after photos of a number of other patients and we’ll review your medical history. Dr. Graham will discuss your goals and the results that are typically achieved through this procedure. He can also answer any questions you may have about combining brow lift with laser resurfacing, eyelid or face lift surgery, or liposuction under the chin.
The operation is done as an out-patient procedure in less than two hours. Long-acting local anesthetics are used in combination with general anesthesia. Post-operative medications include antibiotics, pain medications, and nausea prevention drugs. A bandage is only used overnight. Most patients can return to light work and driving a car in less than one week. We’ll evaluate your progress several times over the first few months following brow lift surgery. The results of this procedure are usually long lasting. Additional Facial Cosmetic Procedures
The following facial plastic surgery treatments offered at our Greenville practice are usually combined with the other procedures described above, but they can also be performed on their own.
Lips often change as the face ages. They decrease in size, fullness, and protrusion (pout). They develop wrinkles and vertical lines. The corners turn down. Lipstick application is more difficult. If you are having surgery and anesthesia, Dr. Graham recommends using your own fat (autologous fat grafting, fat transfer) to enhance lip fullness, and improve the area between the lips and chin. It can produce soft, naturally shaped lips. Restylane® or other fillers may be easier when a small office procedure is desired.
Fat grafting involves removal of fat from your tummy or thigh, processing it gently, and carefully placing very small portions of it in desired areas. Some portion of the fat will be broken down and removed by your body, but generally one-third to one-half remains, probably for many years. Moderate initial over-treatment is typical. This is a procedure that may be planned in two stages to achieve the best fullness.

Recovery takes one to three weeks. Most proponents of fat grafting consider the results after six months to be long lasting, however complete disappearance is possible. Round facial curves are characteristic of healthy, young looking faces. Many patients lose youthful fullness, particularly in the areas beneath the eye sockets and lateral to the nose (anterior malar area). Another affected area is between the chin and the jaw, below the corners of the mouth. Dr. Graham recommends treating this condition with your own fat (autologous fat grafting, fat transfer).

Facial proportions are important to an attractive appearance. If your chin is under-developed, the jaw is not well defined, the lips don’t seem to fit, and the nose looks larger. Chin augmentation with a solid silicone plastic implant may establish more balanced and attractive proportions. The thickness is only millimeters, but it can make great improvements without making you look unnatural.

Neck liposuction may be recommended to improve facial aesthetics. Chin implant surgery is performed while the patient is sedated with local or general anesthesia. A supportive dressing is worn for several days after the procedure. The patient can resume light activities in several days. Neck and chin liposuction is also known as submental liposuction. Ideal candidates have good elastic skin tone, a localized fat collection, and generally are younger than 45 years of age. Some patients are in their twenties, but have inherited an isolated double chin (without being overweight).
During your consultation we will show you a variety of examples of other patients’ results. Chin implants may be suggested for additional cosmetic enhancement. The liposuction procedure will be completed in about an hour under general anesthesia. An elastic support band will be worn for 1-3 weeks after the procedure to facilitate healing. Risks will be discussed and given to you in written form prior to surgery. The contour improvement with neck and chin liposuction is quite significant, but it is important to maintain a stable weight to ensure that results last.
